Please turn in the following documents when applying for transitional housing. Indicate if you need assistance in attaining any of these items by circling items you do not have currently.

  • Social History Packet
  • Release of Information
  • Itemized Checklist
  • Two (2) Most Recent Mental Health Exam’s
  • Photocopy of State ID/ Driver’s License
  • Photocopy of SSN
  • Photocopy of Birth Certificate
  • Copy of School Schedule (if applicable)
  • Copy of High School Diploma or current Grades
  • Medical Passport
  • Proof of Current Medicaid Status/ Medical Insurance
  • Proof of income (check stubs, confirmation letter, YAVFC eligibility, etc.)
